Please find below information for MSK athletes for the upcoming school year, 2017-2018.  All forms and schedule information can be found at:

Athletic Card Online Information
All Middle School Athletes (6th, 7th and 8th graders) will need to fill out the forms below. 

You will need the following information before you start:
1. Your students 5 digit Power School ID
2. Date/Month of their last Tetanus (TD) booster
3. Your Insurance Company, Policy Number and Name on Policy

1. First go to Download and Print.

a. Your child  will need a updated physical. The Medical Release Form is all you need to get signed when you get your child’s physical. If your athlete has had a physical after January 1, 2017, please have your physician sign off on this form or have a signed note from the physician’s office stating that the student is cleared to participate in athletics.

b. Physicals are valid for 2 years from the date of the exam. If you child’s physical is going to expire prior to or during the school year the athletic department will notify you via email.

c. Read the Athletic Handbook and the Heads up on Concussions information. 

2. Second go to Electronic Participation Form

a. Fill in all areas on the Athletic Card. 

b. Both you and your student will need to sign off that you read the Athletic Handbook and the Heads Up on Concussions information.

c. When you finish the athletic card it will allow you to go directly to the Concussion Management page. Please read the policy and then sign off. 

3. You have now completed all online forms. If your student  has an expiring physical you will need to have them bring their physical form to the MSK office or fax it to 207-467-9059.

Protocol for Away Games

Protocol for away games is as follows:

Athletes will get on the bus for away games in the front of our building between 2:15 and 2:30. Athletes should wait for their coach before they get on the bus. Athletes are expected to travel to and from the games by RSU 21 buses. 

In the event that a parent or guardian would like to take their son/daughter home from the away contest, the parent needs to sign their child out with the coach. If a parent wants their child to go home with another parent, they will need to get that in writing to Mr. Coulombe for approval.
