An application must be completed for all athletic and co-curricular activities. The completion of this application is not intended to be a burden on the individuals who prepare the application. Its purpose is to identify the components of the activity, plan out the basics needed to institute the activity, and assure that RSU 21 sponsorship of the activity is authorized.

Students, teachers, and staff are encouraged to be creative and pursue activities that have identified interest and explore out-of-class experiences.

A mentor/sponsor for each program must be identified. The identified individual should be a person who is willing to assist in the development of this application and provide guidance to the students who participate. This individual does not need to have financial responsibility for running the program.

Each rubric sheet must be completed. However, every rubric may not apply to the intended activity. If a rubric sheet does not pertain to the proposed activity, select NA from the Rubric Value and explain briefly why the rubric sheet does not apply. (i.e. A group of students wishes to start an after school Frisbee club. The group does not need equipment, personnel, has no impact on administration, and does not intend to field a competitive team. NA would be selected as the Rubric Value for these items and a brief explanation of the NA would be provided in the Comments section. The group would need to check with the Athletic Department and Facilities to identify an appropriate location to play Frisbee.)
